Of Jackson, passed away late afternoon on Sunday, August 30, 2015 at the age of 72 years. He is survived by his loving wife of 49 years, Patty Kubiak; three children, Michael (Kelly) Kubiak, Brian Kubiak and Holly Kubiak; grandchildren, A.J., Derek, Grace, Ben, Jack, and Kate; and brother, Jerry (Barbara) Kubiak; brothers-in-law, Billy and John (Wendy) Bergstrom and his special sister-in-law, Susie Carr; and many loving nieces and nephews; and Diane too.
He was an U.S. Army veteran, retired from Consumers Energy after 30 plus years of service, loved golfing, fishing, reading, music and was a great cook. A special thank you to his brother Jerry who through his donation of a bone marrow, gave Tom 30 extra years of life, his fishing buddies of 40 years; Fred, Ed and Mike; and his good friends, Ron (Nancy) and Dave (Joann), a special thanks to the hospital staff; 6 West, especially Lindsey, all of the people at ICU especially Ally, Dr. Wadhwa, Dr. Halsey, Dr. O'Neill, and Dr. Murray.
The Mass of Christian Burial will be held at Our Lady of Fatima Catholic Church at 12 Noon on Thursday, September 3, 2015 with Father Timothy MacDonald officiating. Private interment at St. John's Cemetery. The family will receive friends at the church from 11AM-12 Noon.
